The Bridge Film Festival Entry Form

DEADLINE FOR ENTRIES IS FEBRUARY 27, 2009. Please read the full terms of the rules and regulations of the Bridge Film Festival available on our website. REMEMBER...each Friends' school or meeting may submit one narrative, one documentary and one public service announcement per division. The festival is open to any middle or upper school student attending a Friends school or Quaker Meeting. If a Friends school has both an upper and middle school division they may enter up to 6 productions.

Title of Production *

Name of School or Meeting * Each Quaker school and Friends Meeting may enter one narrative and/or documentary film and/or one public service announcement per division.

Faculty or Clerk Advisor * Productions must be approved and submitted with a supervising faculty member at the participating Quaker school or the Clerk of a Friends Meeting.

Student Filmmaker * Name or names of the students most responsible for the production.

Upper School
Middle school

Entry Category * One entry per form submitted.

Narrative
Documentary
Public Service Announcement

Advisor Email Address * The Festival only communicates via the Advisor.

Running Time of Production * Narrative and Documentary films may not exceed 10 minutes. Public Service Announcements may only be 30 or 60 seconds.
