Student Xacti Camcorder & Xacti/MacBook Backpack Permission Form

Please use this form when your students need to check out Xacti Camcorders or Xacti Camcorder/MacBook Backpacks from the library for course assignments. Students may check out camcorders and/or laptops for a two week time period. Be sure to familiarize your students with the Xacti Camcorder and/or MacBook before they check them out. Any questions?
