Parents, please fill out your name below and choose the sizes of the shirts you would like to order for your child(ren). These shirts will be exactly the same as the previous choir t-shirts, so there is no need to order unless your children are new to the choir or have outgrown their shirt. Orders must be placed by Thursday, November 5, 2009. The price is $10 per shirt.
