University Centers tech-fee waiver supplementary application information

Please fill this form out _after_ you have requested and received an estimate of the tech or A/V costs of your event, since the University Centers Advisory Board is only able to help fund your event by waiving some of the tech or A/V costs that your event will incur.

Two of the fields are marked as referring to a past, similar event that your organization may have hosted. The rest of the fields refer to the event for which you are requesting funds.

Please complete all fields in this form as accurately as possible.

NOTE:
(1) You must attend a UCAB meeting to present your request to the board in person, or we will not approve your request.
(2) UCAB generally grants waivers only for events happening University Centers property
