Millersville University Intramural Department: Individiual Sport Form

READ Intructions BEFORE completing form:

1. Complete one (1) roster per individual & sport

2. Sports designated as "Doubles" must have a partner

3. Individual Sport Form MUST be submitted before 9pm the night of the deadline date. EXCEPTION APPLIES TO POKER OR WII TOURNAMENT!!! See further information below in next instruction number for Poker & Wii Tournaments.

4. If you are participating in the Texas Hold 'Em Poker Tourney or Wii Tourney, you will receive an email regarding your reserved entry into the tournament. Since the deadline date is the tournament date, if you want a reserved entry/reseved spot in tournament the Individual Sport Form MUST be submitted before 6pm the night of the deadline date/tournament date. A list of reserved tournament entries will be provided to intramural personnel if you don't receive an email confirmation.

5. All individual sport participants will be contacted when league format has been developed & schedules will be posted online.
