Millersville University Intramural Department: Intramural Team Roster

READ Intructions BEFORE completing roster:
1. Complete one (1) roster per team & per sport

2. Rosters MUST have a minimum number of (6) players for league entry

3. $10 Forfeit Fee MUST BE PAID @ SMC bank before 4:30pm night of captain's meeting/deadline date. After payment you will receive a forfeit fee receipt, you MUST bring forfeit fee receipt to captain's meeting & provided to intramural personnel.

4. Rosters MUST be submitted before 9pm the night of Captain's Meeting/deadline date.

5. All teams MUST have captain/co-captain/team representative attend MANDATORY Captain's Meeting for league entry & scheduling purposes while submitting a roster does not guarantee league or team entry.

6. Prevent scheduling problems: If your playing multiple sports, select different/opposite leagues for each sport! Doing so, elminates double bookings, forfeits & scheduling problems for intramrual personnel.
