INTERNATIONAL DODGEBALL TOURNAMENT REGISTRATION FORM
4TH ANNUAL INTERNATIONAL DODGEBALL TOURNAMENT
Date: Sunday, December 6, 2009 Time: 2:00PM - 6:00PM Place: Walker Fieldhouse Teams: 8-10 players per team Cost: $50 early registration fee per team, $60 on-site registration fee per team
All proceeds from this event will go towards funding "Catalyst Service-Learning Trips" to New Orleans, Chicago, Washington D.C., and White Earth (MN) as well as "Pay it Forward Service Road Trip," for Hamline students.
INSTRUCTIONS:
1) Please complete all required fields and submit your teams registration form by Friday, December 4th. Team who register on-site the day of the tournament pay $60.
2)Team Leaders are responsible for ensuring all team members have read and agree to the "Assumptions of Risk and Waiver of Liability." All participants will be asked to sign the waiver form at check-in the day of the tournament.
3) Cash or Check Payments will be accepted and must be delivered to the Bush Student Center Information Desk. Please indicate your team name when submitting a payment. Make Checks Payable to: Hamline University OSLV.
