Worcester Festival 2009 - Staff Thank You Event Form

Please submit 1 entry for each group of staff members with common contact details attending the meal. If you have a partner who would like to attend the meal with you but who was not a staff member please indicate - this may be possible at a nominal cost subject to numbers attending.

Please note: transport pickup points are indicative - they will be finalised once demand is known - if you would like another stop, please indicate this in the Comments section.
