SMA Sweatshirt Order Form - due by Monday, October 26

Please use this form to order your black SMA hooded pullover sweatshirt for just $20. There will only be ONE order placed--no reorders!

The sweatshirts will be a Hanes 50/50 garment available in Youth M, L, and XL, as well as Adult S, M, L, XL, 2XL and 3XL. To decide which size you want, or to view the design of the sweatshirt, go to www.shannontrial.wordpress.com, click "Photography and Graphic Design," and scroll down to the Hanes sweatshirt size images.
