Ohio Operation: Military Kids (OMK) Program Request Form
The purpose of this form is to assist with requests for the wide range of recreational, social, and educational programs for military youth available through OMK. The purpose of these programs is to foster a network of community support for military youth. Please fill out this form completely so we can plan accordingly to accommodate your request.
