Please complete the form below if you would like to be added to the Simmons Health Center's H1N1 vaccine waiting list. In accordance with the guidelines of the Department of Public Health, students in high risks groups will be immunized before students in the general population. We hope to eventually have enough vaccine for all students, so even if you are not in one of the priority groups listed below you will be included on the list. Staff and faculty are welcome to sign up for the vaccine. After students are immunized we will offer vaccine to staff and faculty as vaccine become available.
You should still sign up for the vaccine even if you have been diagnosed with ILI (influenza-like illness) There is no danger in receiving the vaccine even if you have had H1N1 flu.
