This is a very involved process to get doggies transported to safety, so in order to receive assistance you MUST fill out this form in it's entirety in order to get a completed transport. If there is more than one sending shelter then please fill out a separate form.
Collar AND Leash MUST BE PROVIDED by Sending Shelter as well as enough food for transport and overnights. If young puppies that are not able to be put on ground Sending MUST PROVIDE Crate:
We do have an ongoing fund if you are able to donate a small amount to assist with transports that may need to compensate for gas or boarding efforts, and crates when needed. Please remember that dogs will NOT be transported if showing any signs of illness or without current vaccinations!
