Person Centered Planning Facilitator Certification Application process: 1. Apply for the program a. Fill out this online application b. Review and electronically initial PCP Ethics document (included in this form) c. Review and initial PCP Mentoring information d. Submit application 2. Your Application will be reviewed by the PCP Committee at The Center for Human Development. a. Applicants will be contacted by the committee when accepted to the program b. From the options below select the one that best describes your PCP training & experience level: --New Facilitator: I have successfully completed the HUMS V240 Person Centered Planning course. --Experienced Facilitator: I have completed PCP training either at The Center for Human Development or its equivalent and I have 5+ years of experience in the PCP process.
