Milwaukee Area Skywarn Association - Identification Badge Application

Below are the requirements needed to apply for an identification badge with MASA: * Applicant must have attended a basic storm spotter training program within past 2 years. * Applicant understands that identification badge is valid for 4 years from date of issuance. * Applicant must submit a self portrait (color photo, with dress shirt and tie showing). * Applicant agrees to make a donation to the Milwaukee Area Skywarn Association, Inc. of $10.00 or more. Donations can be made at the following website: http://www.mke-skywarn.org/support.htm Please complete the below form to apply for your MASA photo ID badge.

Credentials

The MASA unit number and amateur radio callsign will not appear on the badge by default. Please add in the comments section at the bottom of the page if you wish to have your callsign added to your badge.

The heading you select and photo you submit at a later time will appear on the badge.
