Requesting Library or Cluster
Enter your SCLS agency code or full library name. If asking on behalf of a cluster, enter the Cluster number.

Select the module or Area that you have a question on.
Public Access Catalog (PAC)
Response time issues
Please describe your problem or question with as much detail as possible. If you are reporting a non-urgent response time issue, include the (approximate) start time, the task(s) you were doing, and the PC number (ex. SCAPC12). Also note if the issue has been resolved.
Please include example item barcodes, patron barcodes, titles, biblionumbers, etc. so SCLS staff can investigate the problem.
Please indicate the level of response needed for your issue
You may determine how important this problem is in relation to other issues. It will help SCLS staff prioritize. If something is truly urgent, for example, you can't access the system at all, please call one of the Help Desk numbers.
High -- please address ASAP
Medium -- this can wait a little bit
Low -- this can wait for a while
Never submit passwords through Google Forms.
